Superfest XXV Entry Forms and Information
The latest version of the 2005 entry form is available, and the call for entries is going out!
The entry form can be viewed in .pdf
(Adobe Acrobat) format, .xls (Microsoft Excel)
format or .html (Web page) format, .
The Acrobat version is the preferred but may require the reader, downloadable
at http://www.adobe.com. The Excel
version can be filled out electronically (on some browsers), then printed.
The HTML version is readable by any browser, but is least likely to
display correctly.
Entry requirements
- Works must be on the subject of disability.
- Work must have been produced within the last 5 calendar years (since January 1, 2000 for entry in Superfest XXV in 2005).
- Entries for Superfest XXV must be postmarked on or before January 31st, 2005.
Because we have to process entries and schedule judges, the deadline is solid.

Discounts are cumulative, so you could save up to $30 on each entry. We do not have any discounts for multiple entries.
Submission checklist
- Complete the 3-page entry form
- Pull the record tabs on the entries.
- (They haven't recorded over an entry yet, but accidents do happen...)
- Place tape in a hard case.
- (We have had submissions arrive damaged in shipping)
- Check all information and totals on the entry form
- Place submission, entry form, entry fee, return postage and a return-address label in a shipping container
- (We recommend a box rather than an envelope, for the same reason as the hard case. The return address label and postage are not necessary if you don't want the entry returned.)
- Address the package for:
- Superfest Video Festival
- P.O. Box 1107
- Berkeley, CA 94701-1107
- Write your return address clearly on the package
- Send it out, and wait for the results!
- (If you would like to confirm receipt of the package, send us an e-mail to superfest @ aol.com (remove the spaces around the @) when your package goes out, and we will reply as soon as we get it at the Post Office)
Final notes:
Results are usually available in early April. Winning submissions will be screened in the San Francisco Bay Area in June, 2005
